Parasitic infections affecting children, specifically enterobiasis, are predominantly associated with the ingestion of pinworm (Enterobius vermicularis) eggs. The onset of infection typically stems from fecal-oral transmission in environments like schools and daycare centers, compounded by incomplete hand hygiene. Epidemiological data indicate that pinworm infections are diagnosed with the highest frequency in pediatric populations. The selection of an appropriate treatment regimen combines targeted anti-helminthic therapy with rigorous household hygiene.

Biology and transmission pathways of pinworms
The pinworm (Enterobius vermicularis) is a small, whitish nematode that parasitizes the human large intestine, particularly the cecum and terminal ileum. Female pinworms migrate to the perianal region at night to deposit thousands of eggs, which triggers the characteristic intense itching.
Infection occurs via the fecal-oral route: eggs are transferred to the child's hands through scratching and subsequently onto toys, clothing, bedding, or food. Due to their lightweight nature, eggs can even remain suspended in indoor dust particles. In community settings such as kindergartens and schools, close social interaction and incomplete hand hygiene facilitate rapid cross-contamination and repeated reinfection cycles.
Clinical symptoms of enterobiasis in childhood
An infection with pinworms may initially present without noticeable symptoms. However, as the parasite burden increases, typical clinical manifestations emerge. The primary symptom is intense nocturnal perianal itching (pruritus ani), directly triggered by the oviposition activities of the female worms.
Secondary clinical features often include sleep disturbances, nocturnal teeth grinding (bruxism), irritability, chronic fatigue, and diminished concentration capacity during daytime activities. In female children, wandering worms may access the urogenital tract, inducing vulvovaginitis, urinary tract infections, or persistent discharge, leading to significant discomfort and pain.
Medical diagnosis and anti-parasitic treatment
Clinical diagnosis relies primarily on detecting parasite eggs using the adhesive tape test (perianal swab), which should be performed in the morning prior to the child's hygiene routine. Standard stool examinations are frequently unreliable for enterobiasis because eggs are rarely shed directly in feces.
Medical management involves targeted anti-helminthic medications (such as mebendazole or albendazole) that inhibit vital metabolic processes in the worms. Because these agents generally eradicate adult worms while leaving eggs unaffected, a second dose must be administered after two to four weeks to destroy newly hatched parasites. Furthermore, simultaneous treatment of all household contacts is mandatory.
Hygiene protocols and supportive home remedies
Pharmacotherapy alone is insufficient unless rigorous household hygiene measures are implemented in parallel. Throughout the treatment period, bed linens, towels, and underwear must be laundered daily at a minimum of 60 °C and thoroughly ironed to destroy persistent eggs.
Essential daily supportive protocols include:
- Thorough handwashing with soap after every toilet use and before meals, combined with keeping fingernails trimmed very short.
- Daily morning showers or washing of the perianal area to mechanically remove eggs deposited overnight.
- Adopting a high-fiber dietary pattern that stimulates intestinal motility, assisting in the natural elimination of parasites.
Through the consistent combination of prescribed medication and strict domestic discipline, enterobiasis can be successfully and permanently resolved.
