Key Takeaways
Urinary root disease is a health issue with an increasing diagnosis rate, particularly among men over forty. Over the past few years, there has been an observed rise in the number of diagnosed cases of the disease in developed countries. The main causes are poor nutrition, lack of exercise, and the intake of certain medications. Without proper treatment, urinary root disease can lead to a significant deterioration in the quality of life or even to disability. Therefore, it is very important to implement appropriate nutritional interventions and introduce healthier eating habits, which can have a significant impact on improving the health of patients.

Understanding Urethritis: A Definition

Urethritis, also referred to as acne, arthritis, or urinary incontinence, is a condition caused by the accumulation of monosodium urate crystals in tissues, organs, and joint fluids. This results in recurrent joint inflammation and severe pain, which is most pronounced in the morning. Historically, urethritis was termed "the king's disease" because it primarily affected individuals with sedentary lifestyles and excessive alcohol intake. In the early stages, symptoms manifest infrequently, typically 1–2 times per year or even every two years. Without treatment, however, the symptoms become more frequent, and the condition progresses to a chronic state.

Gout – Symptoms and Diagnosis

In addition to severe joint pain, patients with gout may experience further symptoms, including: fever, chills, fatigue, skin redness at the affected joint site, and impaired joint function. Gout is diagnosed by analyzing joint fluid to detect uric acid crystals. However, blood tests are more commonly performed to measure uric acid levels – the final product of purine metabolism. These compounds play essential roles in the body, such as encoding genetic information and synthesizing nucleosides. Normal uric acid levels are: premenopausal women – 4.0 mg/dl, postmenopausal women – 4.7 mg/dl, men – 5.2 mg/dl (D. Włodarek et al., 2015).

Factors contributing to elevated uric acid levels

Hyperuricemia is diagnosed when blood uric acid levels exceed 7 mg/dl. This condition can arise from various factors and is classified into primary and secondary types. Primary hyperuricemia is genetic in nature, resulting from impaired enzyme function in purine metabolism, which leads to excessive uric acid production. Secondary hyperuricemia, on the other hand, is caused by external influences such as a diet high in purines, excessive alcohol consumption, the use of certain medications, and impaired kidney function, which hinders the excretion of uric acid.

Dietary Management of Gout – Essential Nutritional Guidelines

Diet plays a crucial role in the development of gout, so upon diagnosis, it is essential to modify the patient's diet. A key aspect is reducing the intake of purine-rich foods. Patient education is vital, as proper dietary management can prevent recurrences. Purines are primarily found in organ meats, processed meats, certain fish, seafood, legumes, and mushrooms. This does not mean these foods must be completely avoided, but rather that their consumption should be limited or replaced with alternatives. Processed meats and organ meats can be substituted with lean meats like chicken or turkey, which contain fewer purines. High-purine fish such as herring, sardines, or mackerel can be replaced with salmon or cod, which have lower purine levels. Fish and seafood are valuable sources of omega-3 fatty acids, so they should not be entirely excluded. Legumes like lentils, chickpeas, or peas should be reduced but not eliminated, as they provide plant-based protein, vitamins, and minerals. Alcohol, particularly beer, should be avoided as it increases uric acid levels. Sugar and sweets can exacerbate symptoms, so foods containing added fructose should be limited. Adequate hydration is important – patients should consume at least three liters of fluids daily, preferably water, tea, or grain coffee. Moderate physical activity can aid weight loss and lower uric acid levels, while intense exercise should be avoided during gout flares.

Gout Prevention – Essential Strategies

Research indicates that gout often coincides with excess body weight and obesity. Implementing a calorie-restricted diet can effectively lower weight, thereby reducing the likelihood of gout development (K. T. Clebak, A. Morrison, J. R. Croad, 2020).

Reduction in the consumption of red meat and organ meats

Organ meats such as liver and spleen contain high levels of purine compounds, making it advisable to significantly reduce their intake. Additionally, it is recommended to limit the consumption of red meat and opt for leaner options like chicken or turkey. According to healthy eating guidelines, healthy adults should not exceed 500 grams of red meat per week. Individuals with gout should further reduce this amount and choose poultry and fish no more than 2–3 times per week.

Cessation of alcohol consumption

Alcoholic drinks, particularly beer, contribute significantly to elevated uric acid levels in the blood. Discontinuing their intake may help prevent the development of gout.

Consuming vegetables and fruits in various colors

These foods contain minimal purines but serve as a rich source of essential vitamins and minerals. Consuming at least 400 g daily helps maintain optimal bodily functions.

The role of dairy products in a balanced diet

It is essential to recognize that fermented dairy products such as natural yogurt, kefir, and quark are not significant sources of purines, and thus should not be excluded from the diet. Consuming two servings of milk or its alternatives, like yogurt, kefir, or quark, is recommended daily. Opting for low-fat and sugar-free options is crucial to maintaining a healthy dietary balance.

Physical exertion

Consistent physical activity lowers the risk of obesity, aids in weight reduction, and decreases the likelihood of gout development. Daily movement is recommended, tailored to the individual's capabilities and health condition.
