Hypercholesterolemia: Key insights into elevated blood cholesterol levels
Elevated serum cholesterol concentrations, clinically termed *hypercholesterolemia*, represent a pervasive public health concern in Poland, with epidemiological estimates suggesting a prevalence exceeding 50% among the adult population. A particularly alarming aspect of this condition is the substantial proportion of undiagnosed cases, as hypercholesterolemia frequently remains asymptomatic in its early stages, with detection typically occurring incidentally during routine lipid panel screenings. Diagnostic thresholds for hypercholesterolemia are defined by total cholesterol levels ≥190 mg/dL (5 mmol/L) and low-density lipoprotein (LDL) cholesterol ≥115 mg/dL (3 mmol/L). Circulating cholesterol originates from both endogenous biosynthesis within cellular systems and exogenous dietary intake. Laboratory evaluations further assess plasma lipoproteins—lipid-protein complexes that facilitate cholesterol transport—categorized as low-density lipoproteins (LDL), high-density lipoproteins (HDL), intermediate-density lipoproteins (IDL), very low-density lipoproteins (VLDL), and chylomicrons. Among these, LDL (colloquially referred to as "bad" cholesterol due to its atherogenic potential in promoting plaque formation within arterial walls) and HDL (termed "good" cholesterol for its role in reverse cholesterol transport from peripheral tissues to the liver for excretion) hold paramount clinical significance. Excessive LDL accumulation fosters lipid deposition in arterial endothelia, culminating in atherosclerotic plaque development, progressive luminal narrowing, and compromised tissue perfusion. Preventive strategies emphasize dietary modification (limiting saturated fats and dietary cholesterol), mitigation of cardiovascular risk factors (e.g., tobacco smoking, hypertension), enhanced physical activity, and patient education on lifestyle optimization. Therapeutic management prioritizes the adoption of a tailored nutritional regimen aligned with evidence-based guidelines; in refractory cases, pharmacologic intervention with lipid-lowering agents (e.g., HMG-CoA reductase inhibitors, ezetimibe, or PCSK9 inhibitors) may be warranted. Critically, hypercholesterolemia constitutes the foremost modifiable risk factor for cardiovascular diseases, including myocardial infarction, cerebrovascular accidents, and coronary artery disease.
