Maintaining a dynamic equilibrium between fluid intake and excretion represents a cornerstone of physiological homeostasis, as both chronic deficiency and excessive fluid retention can precipitate severe metabolic and systemic dysfunctions. The human body undergoes continuous water loss via the urinary system (as urine), eccrine glands (through thermoregulatory perspiration), and respiratory mucosal surfaces (via evaporation). Given the organism’s limited capacity for endogenous metabolic water synthesis and the absence of long-term storage mechanisms, sustained external hydration becomes indispensable. The etiologic spectrum of dehydration spans from mundane causes—such as inadequate fluid consumption—to complex pathophysiological processes associated with chronic conditions, including type 1 and type 2 diabetes mellitus, renal pathologies, and endocrine disorders. Key predisposing factors further encompass excessive dietary sodium intake, iatrogenic use of laxative agents, episodic diarrhea of varied etiologies, recurrent emesis, febrile states, extensive tissue injuries (such as thermal, chemical, or solar burns), exposure to extreme environmental conditions (particularly elevated temperatures), strenuous physical labor or athletic training, high-fiber diets (which impede water absorption), and hyperproteinemic diets (wherein protein catabolism yields only trace amounts of metabolic water, insufficient for effective urea excretion). Additionally, dehydration may arise as an adverse effect of pharmacotherapy (e.g., loop diuretics, thiazides, or psychotropic medications) or excessive ethanol consumption. Prompt therapeutic intervention targeting both causal elimination and symptom alleviation is critical to averting systemic complications. Based on the proportional loss of water relative to electrolytes, three primary dehydration subtypes are distinguished: hypertonic (predominant water deficit with relatively preserved electrolyte levels, commonly observed in febrile conditions), isotonic (balanced loss of water and electrolytes, typical of diarrheal illnesses), and hypotonic (excessive electrolyte depletion relative to water, frequently induced by diuretic overuse).
